2017/01/01 Switched to Medicare Insurance .
Called Apria in early January about insurance change. (It is a prompt on the billing number)
2017-01-20 Received denial letter from Humana for a $44.48 charge from Apria
2017/01/26 at 3:27 called Apria. At first they still had me on Humana. Finally found where I had called and changed. Said it would taka while to go through. Charge was for rent.
2017/02/10 VM 8:17 this am. About insurance. Call 800-554-0459 opt 6. 10:10 returned call. Still had old insurance. Asked when last appointment with Doctor was 12/19/16 with folo scheduled 2/27/17. Said they needed document faxed to 731-424-1083 attn: PPMC Face to Face Evaluation. He was very hard to understand. Also finally got signed up for statements.
2017/02/13 Doctors office faxed the notes.
2017/02/22 Signed for certified mail at post office. Paperwork from Apria. Advanced Beneficiary of Notice of Noncoverage (ABN) (Certified Mail poste date 02/14/17)
2017/02/28 Called Apria billing office. Told that they needed paperwork. Finally found sleep study from 2013/09/12. Medicare requires notes and order from 2017. Informed them appointment scheduled 2017/03/27. They will need following: Full name, DOB, Apria account# 0226AYR697, notes that I’m using and benefiting from machine, how long I have used one, copy of their order and sleep study. Faxed to 888-492-0010. They may grandfather in under old sleep study or require a new one. Until they get Medicare on line I will be charged $44.48 per month. When Medicare picks up that starts a whole new 13 month cycle of rent – even if same machine. Doesn’t matter if previous insurance had paid on it. Decided not to return ABN until discussed with doctor.
2017/03/18 CitiCard charged $107.42 Checked on-line statements, nothing.
2017/03/20 Checked on-line statements again. No change. Called billing 866-505-6365 opt 3- Question bill, opt 2 dispute about charge made card… 10:23 am Waiting for person for 19 minutes then transferred me to some kind of voicemail services mailbox??? Called again. After more than 20 minutes on hold spoke with Kala in Ohio. Charge was full rental since ABN not on file. Pointed out that ABN says estimated cost was $40.56 month. Nowhere does it mention a potential charge of $107.42. We discussed returning the equipment. She would have to file as a Dr. Discharge or as AMA – against medical advice. She put in as Dr. Discharge and when I returned equipment they would have me sign an AMA since no paperwork on file. Advised me to take the ABN in with me for them to fax to appropriate office. Gave me a “local phone #” of 800-476-1899 for the 245 Ellery Court Nashville, TN office. Called to make sure open and see if needed to make an appointment. NOT local. On phone for 58 minutes this time, including hold. Asked her to remove my credit card information from my file.
2017/03/21 Received automated message from Apria saying I needed to return my machine to them as soon as possible to avoid additional charges.
2017/03/21 Citibank On-line. Disputed $62.94 of the charge (107.42 – 44.48 monthly rent). Case# D-732058210317
2017/03/21 Got to 245 Ellery Court Nashville, TN office about 2 pm. Was told that they are not a billing office but can take back machine. She told me that the $107.42 was the retail charge since I did not have the ABN on file. I pointed out that nowhere on the ABN does it say anything about having to have it on file or the monthly rent would be increased. Also told her I had called after receiving ABN paperwork and was told that I would be charged the $44.48 rent until Medicare got the proper paperwork from after the doctors appointment on 3/27/17. Gave her the ABN paperwork, Citibank dispute form and a copy of these notes through 3/20/17. Had to sign a AMA form. Also had to sign a 7 page Sales, Service and Rental Agreement form. When I asked her what it was she said it was the same thing I signed when I received the equipment. I sat in office and tried to read whole thing. I noted on page 1 that the 107.42 charge was disputed. On page 3 I wrote in my preferred contact to be through

On page 6 I starred the first paragraph: “I acknowledge and agree that the Company will give me at least ten (10) notice before charging my card only if such charged Recurring Charges will be more than fifty percent (50%) greater than the Recurring Charges authorized under this agreement.” On the last page I scratched out my credit card number then signed and dated.
2017/04/28 Refund check for $44.48 stating ‘after reviewing your account we have determined you are entitled to a refund’
2017-08-18 Received CitiBank email for charge card not present for – a $44.48 charge from Apria.
2017-08-25 Disputed charge on CitiBank.
2017-08-25 Called 866-505-6365 at 2:38 pm. On hold for about 20 minutes, total call lasted 31 minutes. Spoke with Cheryl P. - determined that the bill was for rental for February 2017 -my credit card payment did not show up in their accounts. Told me to email proof of payment to
[email protected] and to include my name and address as well as my account number. I also asked her to remove my credit card information from their records which she said she did.
2017-08-25 Send email with all information requested.
2017-08-29 Received email stating ‘The payment of 44.48 taken on 2.25.17 posted on 3.2.17 but at that time the balance had been removed so no invoice for it to post to. It sat in suspense until it was refunded by check to you around 4.28.17 so you would have received that back as a refund check. Thankyou. Sandra’
2017/08/29 Replied to email stating ‘Yes. Copy attached. Says after review of account entitled a refund. So what is the issue? Why are you charging it again? Twice now I have requested that my credit card information be removed from your records. If you determine I still owe then you can mail me a statement and detailed explanation. DO NOT CHARGE MY CREDIT CARD AGAIN.Included address
