I have been donating with BioLife in Tempe AZ since they opened. I have a protein/Blood issue that I get a form signed every year by my doctor. I also had neck surgery. I came in to do a physical and then donated I got a call a few days later that they have a new policy and I needed to be cleared from my neck surgery to donate. I took the form to my surgeon and when he called BioLife to ask why they needed it someone on the phone said I DONT KNOW. Not something a surgeon feels comfortable with. I went back in person to BioLife to ask what I can do and he gave me a new form and said my primary doctor can sign it. This is the second time my doctor was filling it out because apparently he didn’t print his name and they couldn’t read it. I will point out his name was on the top that they had typed also they spelt it wrong. I was his last patient and I knew he was in a hurry and I answered the questions on the form he looked them over and signed. I brought the forms in and he said the signatures look different and he spelt his name wrong. I said NOOOOOO you guys typed it wrong and I just came from the doctor’s office. I did say I filled out some of the questions for the doctor and then he signed them. I just came from my doctor and said he could call him. Or if I could get new forms I will have him redo it and fax it from his office. Or fax the same forms I was holding from the doctor’s office if he didn’t believe me. He said he would give the forms to quality control. I got called yesterday and they said because I altered the forms which I DID NOT my doctor signed off on it and it was his bleeping signature! I am deferred from BioLife for good! I am so angry!

So be it if am deferred, which apparently there is a enough plasma. The thing I am really angry at is he accused me of forging and altered a doctor’s note. I care WAYYYYY too much about my health to do something so ridiculous. I also wanted just someone to work with me. You don’t believe me??? Fine!!! Call my doctor and inquire, but no one was willing to do that and frankly was proud and arrogant to tell me I was deferred for life.
