Did not receive the two $50 Costco Cash cards promised

I purchased two Verizon Samsung Galaxy S5 cell phones on 12/7/14. There was a $50 rebate offer on both phones, the rebate forms were sent in 12/8/14. I did receive the accessory package from them but not the Costco cash cards. I have sent two emails. It is well over the 12-14 week timeframe for the cash card. The last I heard from them at the 10 week interval was that it was still in process. Unacceptable.
