rebate

I purchased an I pad air from the Verizon kiosk at Costco and was given paper work for a $100 rebate.
The paper work was submitted well before Christmas (2014) but to date (February 4, 2015) I have received nothing. I have corresponded with Stuart Lee Rebates via email (there is no phone) and get only cursory replies saying they cannot locate my request. I am going to go back to the Costco store that sold the i pad and see if they will honor the rebate. Stuart Lee is a dead end.
