Rebate from Costco/Verizon/Stuart Lee
I, too, have not received my $300 rebate after the "12-14 Weeks" from mailing it in on February 13, 2016. It is for a new line of service with a Galaxy Note5 for $275 Verizon New Device Pament - Galaxy Offer + $25 Bonus Costco cash card.

Rebate Finally Received
I posted on March 17, 2016 and thought it only right that I follow-up with the actual progress. Beyond the time frame reflected on both their website and the original paperwork I received at Costco, I did receive my rebate! A few days following my post here, I rechecked their website hoping for an update. Status indicated rebate issued on March 18, and I received the rebate card on March 26. Honestly, I was a little hesitant about using it; I thought perhaps it wouldn't work, but it did.
I will say I am still a bit confused why Verizon is telling people that there is no affiliation. I've included the letter that accompanied my rebate card. Besides the Verizon logo being brightly displayed, Verizon is also reflected in the return address and again in the boxed area where the card was secured. I'm guessing it's a matter of finding the right person/department within Verizon who could possibly help,
Unfortunately, the process and/or limited communication leaves much to be desired for many. I hope that those who need to see and hear this feedback take appropriate action for the many.

regarding complaints
I wanted to send a response to the complaints listed here after reading through a few of them. I worked for Stuart Lee for a few years processing rebates. This company was awesome to work for, treated their employee’s great and did an excellent job. I only left because my husband was deployed and I moved back to my hometown. They handled millions of rebates without any issues. Just for the record here is how it works.
Stuart Lee only gets paid a "processing fee" if you get your rebate. They get zero if you don't. They only issue rebates once they are approved by the retailer. They do not make the decision on who gets paid and who gets turned down. They do not make the rules on how long it takes to receive your rebate, or the criteria it is based on. All of that is done by the retailer. One of the complaints on here tried to list their phone number in code. They are not trying to hide it. They want to be able to track everything said between them and the customer in writing so they ask you to use the website. Their website is simply to use. All you need to do is enter the retailer where you made your purchase, your zip code and your wireless number. If your rebate doesn’t pop up then contact them by clicking on the button at the bottom of the page that says Where my rebate - Click here. I do know if you submit a request to them on their website they will try to help you. You won't get any help resolving the problem here for sure. Most submittals that do not get paid are for some of the following reasons.
A temporary number was used instead of your permanent number, your rebate submittal was not complete or partially filled out, you sent your rebate to the wrong address, you expected your rebate to show up sooner than the 12-14 weeks listed on your rebate form, you submitted multiple times, deactivated your phone, or did not qualify for the rebate which caused your submission to be flagged and researched. Costco and Stuart Lee rebates promised $200 in cost so cash cards if we purchased two phones and switched service. We figured, since it was Costco it had to be legit. Boy were we wrong. Our service was activated Februaty 20, 1015 and Stuart Lee received our properly completed paperwork and required attachments on March 16, 2015. Their website says they haven't received the forms, the post office says otherwise with three confirmations of delivery for t he three different rebates.
Stuart Lee has ignored 4 emails and unfortunately they don't list a phone Number (likely because of their dishonest business ractices). Costco has ignored two emails and a phone call.
This is ridiculous. How can tw to companies that clearly don't care about customers or ethics still be in business? We're more disappointed in Costco because we had been loyal customers for Years and, until this scam, thought highly of them.
