Did not Receive Rebate

I bought an Apple IPhone 7 from Costco 10/26/2017 with a $125 Costco Cash Card rebate offered from Verizon. I filed the proper paperwork to receive my rebate. Checking the status of my rebate shows my cash card was mailed 2/2/2018. I did not receive the card. I have repeatedly requested mypromochoice to check if the card has been used and reissue if not. I have not had any response from them. Please investigate. Thank you.
