I went to register for UIF on the 22/04/2016 and the lady give me my card with my date (3/06/2016) on when I must come back to sign so they can pay me UIF. I went there on the 3/06/2016 and the guy who help me took my id and go on their system, he then tell me my file is messed up and refer me to that lady's supervisor. I go to that supervisor but he is not very friendly just plain rude. He went in on the system and I quickly look but what a mess my UI-19, letter from my employer, salary info, etc. were all deleted or somebody else info not mine, he tell me to get my documents again and come back on Monday (6/06/2016) so he can see if he can help me. I told him 2 of my documents I must get from Sandton ( my employer) and I will not be able to have it that quick and he said that is my problem. I only got my documents Friday afternoon 10/06/2016 and Wednesday 8/06/2016 I received a sms my UIF claim is rejected. I had all the documents on the 22/04/2016 when I registered and the lady who help me did not do her job properly now they reject my claim, I tried to ask the supervisor where is the file with my info because they must file it somewhere but he did not want to listen and just ignore me telling me I must go and get my documents again.

Case: 10004080349 was the rejection and I cannot be held responsible if a person did not do her job properly. Can somebody plse investigate what really happened with my claim.
