Maternity benefit doc

I applied for maternity benefits last year July each time i go to the department they say my UI19 is not right. I submitted it even by hand but kept on saying they never received it sometimes it is faxed right in front of me . So i would like to know where is the problem.Please help my baby will be a year old in 2 months. I do suspect fraud here something has to be done
