loss atm

I sabavat chandulal bearing scholarship a/c no.4216870122335597 and i lost my ez-pay card 1 year back. I completed all the concerned processes but still i did not get any information. Please help me in this regard by either transferring my 1st year money to another account or to get back my card as soon as possible.
