I consistently hear ... "name and I" when it should be "name and me". Good rule to follow is: if I can stand alone, then grammar is correct but if not, then it is wrong.

i.e. It happened to John and I.

Should be, it happened to John and me....b/c you would not say, it happened to "I", you would say, it happened to "me".

Hallmark, you are not alone...writers on a lot of shows make that basic grammar mistake. Shame on them.
