Claim denied by insurance provider

On February 18, 2018 my wife was suffering with severe stomach pain and I did not understand what it was. After an hour or so, I rushed her to the nearest hospital and I admitted her in the ER. The doctors couldn't get a clarity about her pain. They say it might be appendicitis or a urinary tract infection or due to food poisoning. After few hours, they ran other tests and finally confirmed that she's having a stone in one of her kidneys and since it is a small stone, it will pass in urine some day. They prescribed her medicines for a week and slowly in less than 4 days, she became normal.

Sometime in April 2018, we received a letter from the hospital asking us to pay a bill of $22,529.00 immediately. I called up the hospital and asked them why're we charged although we have a insurance. Then I called up the HCC insurance company and asked them about the bill. They spoke to my wife and asked her to fill the claimants and authorization form and email it back. She did fill the form, signed it and emailed it to the insurance company. We thought the insurance company would take care of it.

But, on October 4, 2018 my received a call from the hospital asking to pay the bill since HCC has denied the claim. I called up the HCC and asked why our claim has been denied. They say Kidney stone is a pre-existing condition! I am shocked. How can one know that they have a stone in their kidney?? And I never met a healthy person in my life who went for a routine kidney stone check-up. I know the insurance guys have a lot of money and they have highly efficient attorneys who can defend the company. But I am not a rich guy. All I have is honesty. I want to fight the company at any cost. All I need is support from people out there.
