200 Rebate

I purchased an iPhone 6 at the Verizon kiosk at Costco on December 24. 2015...I have not received my rebate. That was the reason for purchase...I have tried to contact Stuart Lee rebates....No Communication.. went back to Costco on 5 different occasions. I would like to have rebate.. Its April 2016 . I will never refer this BS to anyone. My first bad experience and my last. Where is my rebate.. 4 1/2months still waiting.
