Misbehavior of Bank employee and not issue ATM card and Cheque book

I am New account hoder in Jila sahkari bank Janjgir.
I was fill Up requistion form for ATM card and Cheque book.
Form verified by Branch manger on 12 october 2018
Since we visit bank 4-5 times
But till date not given me ATM card by bank employee and He (bank employee)misbehave with my Husband.
Also He demand Rs5000/- for issue ATM card
