Employer deduct UIF from me and making excuses to give me my form to claim my UIF

Good Day

I hope you can help me. I've worked for this company and they have retrenched me and now I'm requesting them the UI19 form to go and claim my UIF, but it seems like they never register the company for UIF but they've deducted the amount from my salary every month.

Will you please be so kind as to assist me in this matter and guide me on what to do and where to go.
