I introduce myself Amit Shukla is registered pharmacist with Reg No 23008 in MP. While I checked my personal details in database of Registered Pharmacist in MP, my name is incorrect. It seems incorrectly name entered in database by data entry operator. Request you to please help us out to resolve this problem.

My correct name is Amit Shukla
Incorrect name entered in pharmacy.mponline.gov.in under my registration number 23008.

Please let me know whom to approach since I already submitted my graduation mark sheets and provision certificate to pharmacy office at Bhopal and office incharge has not given any acknowledgment receipt for the same.

Please find attached my original pharmacy registration license with correct name (Amit Shukla) and renewal pharmacy registration with incorrect name as Amit Sharma.
