I purchased my apple 6-S on December 18, 2015, and as of May 3, 2016 still have not received my cash rebate. All the necessary forms were sent to this company within 24 hours after buying this phone.

Purchased at Costco-Maria Del Rey, CA.

On December 18, 2015 I purchased 1 Apple 6-S with the promise of $100.00 cash card rebate. It was bought at Costco in Marina Del Rey, CA. I have submitted the necessary forms to Stuart Lee in Lynnwood, WA, but when I click send, I am asked to do this all over again. Three times later and I still cannot make contact with this company. Please help.
