never received rebate for cell phone

Purchased GS5 cell phone from Wireless Advocates at Costco in Union, NJ on 11/22/2014. Sent in paperwork seperately for 2 rebates: $25 and $50. to Stuart Lee Rebates. Saw online that they received both on 12/3/2014. I received cash card for $25 in February 2015. When I checked status of $50 rebate on Stuart Lee Rebate web site nothing shows. I never received the $50. I
