Rebates

I purchased 4 I phones form Costco Gaithersburg T-Mobile kiosk and suppose to get $ 25.00 rebate for each I phone but so for it'd been over a 1 year I received only one $ 25.00 rebate and rest 3 were denied sent an email to Stuart lee for the rebate status by my T-Mobile kiosk dealer but they never responded back . Please help me finding these rebates.
