I filled a claim last night 07/28/2019 @ or around 930pm,
I purchased this card on the 24th of
July 2019.
I activated the card yesterday 28th and used it for my Lyft app.
I took my Lyft and it cost
$9 and change .
When I was ready to return home my balance said 22 cents,
I immediately called the 800 number on my vanilla card and I was inform there was a $40 charge for (sq-my business) I have no idea what type of business that is and I did not purchase anything online,
and my card was with me the entire time.
No one other than myself has access to my cards or personal information.
Please send me whatever forms or documents that I need to fill out in order for me to process my claim and get my
funds /card return back to me.
I need my money back as soon as possible.
I work very hard for my money.

Can you please help me or direct me to someone who can.

Please see attached copy of my vanilla card front/ back along with receipt of purchase and my driver's license.
